The MO-604 Coordinated Entry System (CES) is committed to ensuring fair, consistent, and transparent access to housing and supportive services for all participants. Individuals or agencies who believe they have been treated unfairly, denied services, or experienced a violation of CES policies have the right to file a grievance.
Grievances may be submitted by using this form to the Coordinated Entry Lead Agency, outlining the concern, relevant details, and any supporting documentation. All grievances will be reviewed promptly, and a response will be provided within a reasonable timeframe. The process ensures that all parties are treated with respect, that confidentiality is maintained, and that issues are addressed in a timely and equitable manner.

Before submitting a grievance to the MO-604 Coordinated Entry System (CES), individuals must first follow and complete the grievance or appeal process outlined by the specific program or agency involved. If the concern is not resolved at the program level, or if the issue directly involves CES operations or decisions, a formal grievance may then be submitted to the MO-604 CES Lead Agency in accordance with CoC policy.

Given the limited availability of resources, the standardized assessment tool also helps prioritize individuals and families with the most urgent housing needs. Trained direct service staff from various community providers administer this assessment, and households are referred to appropriate programs as openings become available.
Completing this assessment is the first step towards enrolling in Coordinated Entry; however, it does not guarantee immediate housing or financial assistance.
